The NICU Care Manager (RN) develops, assesses, and facilitates complex care management activities for primarily physical needs members within the NICU Care Management Team to provide high quality, cost-effective healthcare outcomes. This role involves creating personalized care plans and educating members and their families.
Responsibilities
- Evaluate member needs, barriers to care, and social determinants of health to recommend and facilitate optimal outcomes.
- Develop ongoing care plans/service plans and collaborate with providers to identify resources for members' unmet needs.
- Identify problems/barriers to care and provide appropriate care management interventions.
- Coordinate between the member, family/caregivers, and care provider team to ensure person-centered care.
- Provide ongoing follow-up and monitoring of member status and progress towards care plan goals.
- Offer resource support for local services such as employment, housing, and independent living.
- Facilitate care management and collaborate with providers to ensure timely access to care.
- Conduct telephonic, digital, home, and/or other site outreach to assess member needs.
- Collect, document, and maintain member information and care management activities to ensure compliance.
- Provide education to members and their families/caregivers on disease processes, care gaps, and healthcare benefits.
Requirements
- Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
- 2-4 years of related experience
- RN - Registered Nurse - State Licensure and/or Compact State Licensure
