This role coordinates care and promotes efficient patient flow for mothers and babies within the Maternal Newborn continuum, focusing on discharge planning and post-discharge support. The coordinator assesses risk factors, provides resources, and gathers data to improve patient care and ensure compliance with healthcare standards.
Responsibilities
- Promote throughput and care coordination for the Maternal Newborn continuum of care
- Provide direct and indirect care to mothers and babies before and after discharge
- Review individual charts and round on all discharged patients to assess risk factors and determine post-discharge plan
- Determine eligibility for, complete, and submit Postpartum Infant and Maternal Referral (PIMR) forms
- Provide birthing parents with information and resources related to their specific needs and risks
- Incorporate HCAHPS understanding into patient interactions regarding discharge, transitions of care, and medication communication
- Provide the Perinatal Program Manager with patient feedback data to inform improvement opportunities
- Assess Mother Baby and Labor and Delivery census for eligible high risk inpatients requiring a PIMR
- Develop JIBS for the work of the Discharge Coordinator
- Orient interested staff to the role and collaborate on coverage schedules
Requirements
- At least one (1) year experience in Mother Baby or Postpartum Nursing
- Excellent assessment and effective communication skills
- Current Maryland State license as a Registered Nurse
- Current Healthcare Provider BLS certification from an affiliated American Heart Association training site
- Current NRP certification from an affiliated American Heart Association training site
Qualifications
- Associates Degree in Nursing, Bachelor’s Degree in Nursing preferred
- At least one (1) year experience in Mother Baby or Postpartum Nursing
