The MDS Coordinator RN plays a vital role in ensuring top-quality care for seniors by coordinating MDS assessments in compliance with federal guidelines and providing direct resident care. This role involves collaborating with an interdisciplinary team to maintain a warm, home-like environment while supporting residents, families, and visitors.
Responsibilities
- Coordinate MDS assessments per federal guidelines
- Provide direct resident care to assure quality of life in a home-like environment
- Offer additional support to family and visitors
- Maintain professional behavior as an interdisciplinary team member meeting state and federal guidelines
- Ensure all necessary written documents like Admission Nursing Assessments, Nursing Notes, and Over-All-Care Plans are current in the resident record system
- Assist director of nursing in keeping facility standards up to state and federal and ASI requirements and assisting with all inspections
- Assist in planning, evaluating, and providing a safe home-like environment and quality of life for residents
- Determine that the medication system meets the needs of the resident and is set up according to State regulations
- Keep administrator informed of significant changes in residents conditions, transfers, expirations, or pertinent information regarding residents and/or employees
- Assist in determining the type of residents to be admitted to the facility using established nursing care criteria
Requirements
- Graduate of accredited school of nursing
- RN license
- Experience in MDS 3.0
- Experience in skilled nursing
- Comply with federal and state guidelines, including all licensing requirements
- Knowledge of Medicare, RUG's, state and federal regulations
Qualifications
- Graduate of accredited school of nursing (RN)
- 1+ years clinical experience in Skilled Nursing Facility (SNF) preferred; 2+ years in long term care facility or assisted living preferred; experience in MDS 3.0 and skilled nursing
Nice to Have
- 1 or more years clinical experience in SNF
- 2 or more years in long term care facility or assisted living
