This role involves providing direct nursing care to residents, including treatments, under the direction of a physician. You will supervise nursing assistants and ensure the highest quality of care is maintained according to federal, state, and local standards.
Responsibilities
- Provide direct nursing care to residents, including treatments, under medical direction
- Supervise day-to-day nursing activities performed by nursing assistants
- Ensure compliance with facility policies and procedures
- Participate in facility surveys by government agencies
- Admit, transfer, and discharge residents as required
- Complete accident/incident reports as necessary
- Maintain the daily report
- Perform administrative duties such as completing medical forms and charting
- Receive and record telephone orders from physicians
- Administer medications as ordered by physicians
- Ensure accurate controlled drug records and count controlled drugs with oncoming shift
- Participate in orientation of new employees to the shift
- Attend and participate in in-service education programs
- Monitor nursing personnel for safety standards and infection control compliance
- Maintain confidentiality of all resident care information
- Comply with and ensure personnel respect resident's rights
- Assess basic physical, emotional, and social needs of residents
- Collect data for admissions assessment
- Develop and update resident's comprehensive care plans
- Assist residents with activities of daily living and encourage self-care
- Perform therapeutic nursing techniques and skills as per clinical assessment
- Provide nursing care in conformance with established standards and medical regimen
- Maintain accurate records of observations, nursing care, and resident responses
- Teach residents and significant others good health habits
- Transcribe doctor's orders accurately on MAR
- Chart medications accurately
- Make significant resident observations and communicate to the health care team
- Support resident's expression of quality of life, rights, and dignity
- Manage the care of residents on the unit
- Recognize and respond to changes in resident's conditions in a timely manner
- Communicate with doctors regarding changes in resident's condition and accept telephone medical orders
- Maintain standards for compliance with NYS DOH LTC survey and participate in survey process
- Participate in interdisciplinary team resident assessment meetings
- Complete incident reports and contribute to investigations
- Assign personnel and direct day-to-day unit function
- Participate in documentation of monthly summaries
- Report unit and resident conditions and seek guidance from nurse manager and supervisor
Requirements
- Current, unencumbered, active license to practice as an LPN in New York State
- Ability to make independent decisions
- Knowledgeable of nursing and medical practices and procedures, laws, regulations, and guidelines pertaining to long term care
- Leadership and supervisory ability
Qualifications
- Completed a practical nursing program approved by the New York State Education Department
