About This Role

Provide direct nursing care to acutely and/or chronically ill patients in their homes, working under the supervision of a Registered Nurse. This role involves recording clinical notes, coordinating patient care, and ensuring high-quality patient care in the assigned geographic service area.

Responsibilities

  • Provide high quality clinical services within the scope of practice, as defined by state laws governing the practice of nursing
  • Deliver care in accordance with the plan of care, and in coordination with other members of the health care team
  • Follow all applicable infection control procedures
  • Document patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record
  • Utilize the agency's electronic visit verification program (as applicable)
  • Actively participate in the agency's care coordination process
  • Timely report patients' needs and changes in condition
  • Attend patient case conference
  • Communicate effectively to the patient, family, physician, other clinicians, agency management staff, and case coordinator, community case managers (as applicable)

Requirements

  • Current and unrestricted Licensed Practical Nurse licensure in state of practice
  • Current CPR certification
  • Current driver's license and vehicle insurance
  • Access to a dependable vehicle, or public transportation
  • Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client

  • Home care experience
  • Ability to work flexible hours as required to meet identified client needs
  • Ability to manage multiple tasks simultaneously
  • Able to work independently
  • Good communication, writing, and organizational skills
