Provide direct nursing care to acutely and/or chronically ill patients in their homes, working under the supervision of a Registered Nurse. This role involves recording clinical notes, coordinating patient care, and ensuring high-quality patient care in the assigned geographic service area.
Responsibilities
- Provide high quality clinical services within the scope of practice, as defined by state laws governing the practice of nursing
- Deliver care in accordance with the plan of care, and in coordination with other members of the health care team
- Follow all applicable infection control procedures
- Document patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record
- Utilize the agency's electronic visit verification program (as applicable)
- Actively participate in the agency's care coordination process
- Timely report patients' needs and changes in condition
- Attend patient case conference
- Communicate effectively to the patient, family, physician, other clinicians, agency management staff, and case coordinator, community case managers (as applicable)
Requirements
- Current and unrestricted Licensed Practical Nurse licensure in state of practice
- Current CPR certification
- Current driver's license and vehicle insurance
- Access to a dependable vehicle, or public transportation
- Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client

- Ability to manage multiple tasks simultaneously
- Able to work independently
- Good communication, writing, and organizational skills
