LPN, Long Term Care

About This Role

Provide professional nursing care to residents of all ages in a long-term care facility under the supervision of a registered nurse. Ensure resident health, comfort, and safety by administering medications and treatments, and documenting interactions.

Responsibilities

• Provide professional nursing care for residents of all ages in long term care under the supervision of a registered nurse, advanced practice provider, or physician
• Administer medications or treatments as ordered
• Document resident interactions and outcomes related to resident care, medication, behavior, activity, and dietary
• Establish and maintain effective working relationships with residents, health care providers, and the public
• Communicate clearly and maintain confidentiality of sensitive information
• Counsel residents, family members and/or resident advocate, under direction, on prevention and treatment plans
• Perform other related duties as assigned

Requirements

• Graduate from an accredited practical nursing program
• Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges
• Attended a pre-licensure nursing program that included supervised clinical experience across the life span OR a minimum of 400 hours of experience as LPN at another facility

Qualifications

• Graduate from an accredited practical nursing program, Associate degree in practical nursing preferred
• Entry level with a minimum of 400 hours of experience as LPN at another facility or supervised clinical experience during pre-licensure program
