This role involves contributing to patient care by assessing, planning, implementing, and evaluating nursing processes under RN supervision. The LPN will administer medications, assist in procedures, and document encounters to provide high-quality care to vulnerable patients.
Responsibilities
- Assess patients' physical, psychological, and social dimensions in Health Care and housing units under RN supervision
- Contribute to individualized treatment planning and consult with RNs and staff as needed
- Implement treatment programs directed by Health Care Practitioners
- Administer medications, including IV therapy (if certified)
- Perform diagnostic tests according to established protocols and procedures
- Collaborate in controlled substance counts at the beginning of each shift
- Adhere to safety techniques, procedures, and standard precautions
- Respond to codes or health emergencies within standard guidelines
- Assist in medical or minor surgical procedures
- Implement nutrition and therapeutic diet plans
- Document nursing encounters using SOAP charting
- Ensure effective communication with nursing staff, physicians, and other relevant personnel
- Attend mandatory staff meetings and training sessions
Requirements
- Current licensure as a Licensed Practical/Licensed Vocational Nurse within the state of employment
- Ability to obtain and maintain CPR certification
Qualifications
- Graduate from an accredited school of nursing
- Minimum of one (1) year clinic experience preferred
