This role involves providing quality, individualized health care management to individuals with developmental disabilities, ensuring their optimal health. The LPN will conduct routine site visits, assessments, and medical training for staff, acting as a vital health resource.
Responsibilities
- Ensure each individual receives quality, individualized health care management
- Conduct routine site visits, checking vitals, doing assessments
- Counsel on medication administration and document progress
- Act as a health/medical resource for staff, training non-medical staff
- Conduct audits and reviews of medications
- Ensure compliance with state requirements and organizational policies regarding medical care and records
Requirements
- Current LPN license in the State of Indiana
- Ability to work cooperatively with staff, interdisciplinary team members, and outside agencies
- Strong verbal/written communication skills
- Ability to work independently and efficiently
- Organizational and computer skills
- Successful background clearance
Qualifications
- 1-2 years experience working with individuals with disabilities or other long-term care is preferred
