Provide direct and indirect skilled nursing care to adult and geriatric residents in a long-term care facility. Collaborate with the interdisciplinary team to ensure comprehensive and age-appropriate patient outcomes.
Responsibilities
- Provide direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse
- Maintain collaborative relationships with nursing staff, physicians, and ancillary staff
- Maintain knowledge and clinical expertise through continuing education
- Collect data relevant to resident's physical and psychosocial condition and communicate findings to the Interdisciplinary Team
- Contribute to the creation of a developmentally age-appropriate and culturally relevant plan of resident care
- Recommend revisions to the plan of care after evaluating resident outcomes and identifying referral needs
- Collaborate with residents/families and the Interdisciplinary Team
- Participate in the mentoring/orientation of staff
Requirements
- NY LPN Licensure
- BLS (cert through AHA or RQI specifically)
- 1 year experience as LPN
- Experience serving in an LPN capacity at LTC Facility
Qualifications
- 1 year experience as LPN, specifically in a Long Term Care (LTC) facility
