Provide personalized, one-on-one home health nursing services to patients, helping them regain independence and manage chronic conditions or recover from injury. You will collaborate with physicians and clinicians to develop and implement individualized care plans.
Responsibilities
- Provide professional nursing services under the direction of a Registered Nurse, adhering to state regulations and company policies
- Administer skilled nursing interventions for patient illness, rehabilitation, and preventive care, using a holistic approach
- Utilize problem-specific therapeutic interventions, teaching, and training activities in accordance with the care plan
- Apply clinical knowledge and skills to facilitate problem resolution and achieve personalized patient goals
- Confer with the RN Supervisor regarding changes in the Plan of Treatment and communicate physician orders where permitted
- Implement individualized, goal-driven nursing plans of care
- Maintain timely communication with patients, physicians, and the healthcare team regarding patient needs and status changes
- Participate in care coordination activities and discharge planning as appropriate
- Maintain the highest standards of professional conduct regarding confidential patient information
- Prepare, submit, and maintain required documentation, with visit/shift notes documented on the day services are rendered
Requirements
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing
- Current nursing license in the practicing state
- Valid driver's license
- Auto insurance
- Reliable transportation
- Current CPR certification
- Two years experience as an LPN/LVN in a clinical setting
Qualifications
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing
- Two years experience as an LPN/LVN in a clinical setting, preferably in home health or hospice
