Licensed Vocational Nurse, Home Health

About This Role

Provide personalized, one-on-one nursing care to CenterWell Home Health patients, assisting them in regaining independence in their homes. Work under the direction of an RN to implement care plans and support patient recovery.

Responsibilities

• Provide professional nursing services under the direction of a Registered Nurse in compliance with state regulations and company policies
• Administer skilled nursing interventions for patient illness, rehabilitation, and preventative care using a holistic approach
• Apply clinical knowledge and skills to facilitate problem resolution and achieve individualized patient goals
• Confer with the Registered Nurse Supervisor regarding changes in the Plan of Treatment
• Accept verbal orders from physicians where permitted by state law and communicate them to the Registered Nurse Supervisor
• Utilize a systematic, goal-driven approach in implementing the nursing plan of care
• Maintain timely contact with patients, physicians, and other healthcare team members regarding patient needs and status changes
• Participate in care coordination activities and discharge planning as appropriate
• Maintain professional conduct regarding confidential patient information
• Prepare, submit, and maintain required documentation, with visit/shift notes documented on the day services are rendered

Requirements

• Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing
• Current nursing license in the practicing state
• Valid driver's license
• Auto insurance and reliable transportation
• Current CPR certification
• Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting

Qualifications

• Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing
• 2 years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting
