Home Health Licensed Vocational Nurse

About This Role

As a CenterWell Home Health nurse at Humana, you will provide personalized, one-on-one skilled nursing care to patients in their homes. You will implement individualized care plans, educate patients and families, and maintain accurate records to support patient recovery and independence.

Responsibilities

• Provide skilled nursing care to patients, working onsite in their homes, under the direction of an RN
• Implement individualized care plans in collaboration with the patient, family, and healthcare team
• Educate patients and their families on disease management, treatment options, and self-care techniques
• Maintain accurate records of patient care, including documentation of patient visit, interventions, and outcomes
• Maintain contact with patients, physicians, clinical manager(s), and other members of the healthcare team promptly regarding patient or family needs and status changes
• Participate in care coordination activities and discharge planning

Requirements

• Current nursing license in the practicing state
• Valid drivers license, auto insurance and reliable transportation
• Current CPR certification
• Two years experience as an LPN/LVN in a clinical setting

Qualifications

• Two years experience as an LPN/LVN in a clinical setting

Nice to Have

• Nursing experience in a Home Health or Hospice setting
