Licensed Practical Nurse, Home Health

About This Role

Provide personalized, one-on-one home healthcare as a Licensed Practical Nurse under the direction of a Registered Nurse. Deliver skilled nursing interventions, manage care plans, and help patients regain independence in their homes.

Responsibilities

• Provide professional nursing services under the direction of a RN in compliance with state Nurse Practice Act and company policies.
• Deliver skilled nursing interventions for illness, rehabilitative needs, and preventative care.
• Utilize a holistic approach with therapeutic interventions and teaching activities in accordance with the plan of care.
• Apply clinical practice standards to facilitate problem resolution and achieve individualized patient goals.
• Confer with the RN Supervisor regarding changes in the Plan of Treatment and communicate physician orders.
• Maintain contact with patients, physicians, clinical managers, and healthcare team regarding patient needs and status changes.
• Participate in care coordination activities and discharge planning.
• Maintain the highest standards of professional conduct regarding confidential patient information.
• Prepare, submit, and maintain documentation as required, with visit/shift notes documented on the day services are rendered.

Requirements

• Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing
• Current nursing license in the practicing state
• Valid drivers license
• Auto insurance and reliable transportation
• Current CPR certification
• Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting

Qualifications

• Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing
• Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting.
