Licensed Practical Nurse, Home Health

About This Role

As a CenterWell Home Health LPN, you'll provide personalized, one-on-one nursing care to help patients regain independence in their homes. You will work closely with a dedicated team of physicians and clinicians to develop and manage care plans, utilizing a holistic approach to problem-specific therapeutic interventions, teaching, and training activities. This role involves skilled nursing interventions, rehabilitation, and preventative care.

Responsibilities

• Provide professional nursing services under the direction of a Registered Nurse in compliance with state Nurse Practice Act
• Provide skilled nursing interventions for patient illness, rehabilitative needs, and preventative care
• Utilize a holistic approach in problem-specific therapeutic interventions, teaching, and training in accordance with the plan of care
• Apply knowledge and skills in accordance with accepted clinical practice standards to facilitate problem resolution and achieve individualized patient goals
• Confer with the RN Supervisor regarding needed changes in the Plan of Treatment and communicate physician verbal orders
• Utilize a systematic, individualized goal/outcome driven approach in implementing the nursing plan of care
• Maintain timely contact with patients, physicians, and other healthcare team members regarding patient/family needs and status changes
• Participate in care coordination activities and discharge planning as appropriate
• Maintain the highest standards of professional conduct in relation to confidential information
• Prepare, submit and maintain documentation as required by the Company, with visit/shift notes documented on day services are rendered

Requirements

• Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing
• Current nursing license in the practicing state
• Valid driver's license, auto insurance and reliable transportation
• Current CPR certification
• Two years experience as an LPN/LVN in a clinical setting

Qualifications

• Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing
• Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting
