Licensed Practical Nurse, Home Health

About This Role

Provide personalized, one-on-one care to patients in their homes as a Home Health Licensed Practical Nurse, helping them regain independence and manage chronic conditions or recover from injury/illness. Work collaboratively with a dedicated healthcare team.

Responsibilities

• Provide professional nursing services under the direction of a Registered Nurse in compliance with state regulations and CenterWell Home Health policies.
• Deliver skilled nursing interventions for patient illness, rehabilitation, and preventative care, utilizing a holistic approach.
• Apply clinical knowledge and skills to facilitate problem resolution and achieve individualized patient goals.
• Confer with the RN Supervisor regarding changes in the Plan of Treatment and communicate physician orders.
• Implement the nursing plan of care using a systematic, individualized goal/outcome-driven approach.
• Maintain timely contact with patients, physicians, clinical managers, and healthcare team members regarding patient needs and status changes.
• Participate in care coordination activities and discharge planning as appropriate.
• Maintain professional conduct regarding confidential information, sharing only when access is established and in the patient's best interest.
• Ensure adherence to professional standards for supplies, materials, and equipment needed to safely and effectively implement the plan of care.
• Prepare, submit, and maintain required documentation, with visit/shift notes documented on the day services are rendered.

Requirements

• Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing
• Current nursing license in the practicing state
• Valid drivers license
• Auto insurance
• Reliable transportation
• Current CPR certification
• Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting

Qualifications
