About This Role

Deliver compassionate, personalized home healthcare to patients in the comfort of their homes, collaborating with registered nurses and the care team to manage medical needs and promote independence.

Responsibilities

  • Provide nursing care in the home, including wound care, medication administration, and support during procedures
  • Document assessments, interventions, and outcomes clearly and thoroughly to support safe, continuous care
  • Monitor patient status and alert the care team of any changes to ensure timely, coordinated responses
  • Educate patients and families to support medication adherence, strengthen understanding of health conditions, and promote greater independence at home
  • Collaborate closely with RNs, providers, therapists, and care coordinators to deliver high-quality, personalized care in the home setting

Requirements

  • Vocational School Graduate
  • Licensed Practical or Vocational Nurse (LPN/LVN)
  • Driver License (DL)
  • Basic Cardiac Life Support (BCLS) obtained within 30 days of employment start date
  • 1 year of experience
