Licensed Practical Nurse

About This Role

This role is for a Licensed Practical Nurse to provide empathetic and compassionate care to patients with end-stage renal failure or chronic kidney disease in an outpatient dialysis center. The LPN will build long-term relationships with patients and families and work collaboratively within a supportive team environment.

Responsibilities

• Build meaningful and long term relationships with patients and their family in an intimate outpatient setting
• Deliver care to patients dealing with multiple co-morbidities requiring unique treatment plans
• Leverage a broad range of nursing skills and knowledge to care for patients
• Commit to improving patient health through clinical goal setting and quality improvement initiatives

Requirements

• Current LPN-LVN license in the state of practice
• Current CPR certification
• High school diploma or equivalent
• Ability to work mornings, evenings, weekends, and holidays
• Functional proficiency in computer systems of DaVita within 90 days of hire

Qualifications

• High school diploma or equivalent required
• Minimum of one year's experience in a hospital or clinical setting preferred, but no dialysis experience required

Nice to Have

• One year's experience in a hospital or clinical setting
• BONENT or CHT certification
• Basic computer skills and proficiency in MS Word and Outlook
