About This Role

This role involves assisting the Patient Care Manager in coordinating a team of staff to ensure high-quality hospice care for patients. It is a full-time position for Home Health LPNs doing home visits in Hospice Care.

Responsibilities

  • Ensure the team provides quality care efficiently, adhering to physician orders and State/Medicare guidelines
  • Prepare patient lists and documents for weekly case conferences
  • Provide direct patient care in coordination with the health care team, adhering to scope of practice, plan of care, and physician orders
  • Assist the Patient Care Manager in documentation review/audits to ensure complete, appropriate, and accurate submission by field staff

Requirements

  • Current unrestricted LPN licensure in state of practice (NC required)
  • Current CPR certification
  • Current driver's license
  • Vehicle insurance
  • Access to a dependable vehicle or public transportation
  • Ability to function in any home situation regardless of client characteristics

Nice to Have

  • Home care experience
  • Ability to work independently
  • Good communication, writing, and organizational skills
