Home Health: Licensed Dietitian

About This Role

Provide home health dietitian services to clients, developing and adjusting treatment plans based on evaluation of their rehabilitative status and response to treatment. Coordinate care with the interdisciplinary team and ensure accurate documentation.

Responsibilities

• Observe and evaluate treatment effect and client's physical/emotional response to treatment goals
• Update treatment plans based on evaluation and new orders
• Assess rehabilitative status of the client and develop appropriate treatment plan
• Update and modify treatment plan on an ongoing basis
• Establish rapport with clients and maintain professional communication on the team
• Plan goals and care, communicating with the Interdisciplinary Team (IDT)
• Observe, record, and report information on the client's condition to the attending physician and in the client's medical record
• Document client treatment and response to treatment accurately
• Honor client confidentiality, rights, privacy, and reporting maltreatment expectations
• Follow all Emergency and Response Procedures
• Identify home safety issues and notify RN Case Manager or Clinical Manager of concerns
• Demonstrate proper procedures for handling, lifting, and transfers

Requirements

• Current and active Dietitian license in the state of Minnesota
