Lead the Child and Family Team (CFT) to support at-risk youth in staying at home, in school, and out of trouble. Coordinate team activities and provide direct support using a strengths-based approach to meet the unique needs of youth and families.
Responsibilities
- Build and lead the Child and Family Team (CFT)
- Coordinate team activities to achieve goals for youth
- Provide direct support and education to families and team members
- Guide families in accomplishing objectives outlined in the Family Plan
- Ensure accurate documentation in the UNCG database
- Embody the Wraparound philosophy of "Whatever it takes" to meet unique needs
Requirements
- Bachelor’s degree in Social Work, Psychology, or related Human Services field
- Meet NC Qualified Professional requirements
- Experience with the population served
- Valid driver’s license
- Current auto insurance
- Reliable vehicle for work-related travel
