Provide person-centered support in home and community settings for individuals with intellectual and developmental disabilities (ID/DD), helping them with daily living skills, community integration, and social development.
Responsibilities
- Deliver training, support, and assistance to clients in areas of personal care, recreation, daily living skills, social skills, health needs, personal preferences, individual rights, community integration, and behavior management
- Participate in the implementation and monitoring of clients' individualized support plans, providing observations and opinions to tailor achievable plans
- Accurately complete required documentation including clocking in and out, weekly progress notes, and communication with family and office staff
- Ensure client health and safety at all times, including abuse prevention, performing CPR, and Standard First Aid
- Work directly with clients and their families to ensure client goals are being met and worked towards
- Assist with cooking, cleaning, medication assistance, scheduling, attending medical appointments, and shopping
- Document progress notes with each shift
Requirements
- High school diploma or equivalent
- 1 year previous personal support experience
- Ability to transport individuals
- Certified in CPR and First Aid (prior to clocking a shift)
- Overall competency in working under pressure
- Experience in providing general care and support
- Knowledge of home care services
- Proficient in written communication
- Proficient in oral communication
- At least 18 years of age
- Understand confidentiality requirements and safeguarding individual information
- Literate and capable of understanding written and oral orders
- Able to communicate with individuals, health care providers, case managers, and others
- Able to respond to emergency situations
- Valid Driver's License
- Auto Insurance
- Reliable transportation
- Smartphone
Qualifications
- High school diploma or equivalent
- 1 year of previous personal support experience
