The Licensed Vocational Nurse (LVN) provides skilled nursing care and support to assigned hospice patients under the direction of a Registered Nurse. This role involves performing prescribed medical treatments, pain management, symptom control, and educating patients and families on care techniques and disease progression.
Responsibilities
- Provide intermittent visits and/or shifts of skilled nursing care to assigned hospice patients
- Perform prescribed medical treatments, including pain management and symptom control
- Conduct nursing observations and teach patients/families about disease course and care techniques
- Participate in care plan development and implementation with the interdisciplinary team
- Document patient care using electronic medical records
Requirements
- Current LVN license within the State
- At least one-year experience as LVN/LPN
- Valid driver's license and auto insurance
- Reliable/well-maintained transportation
- Sympathetic attitude toward caring for hospice patients and families
- Ability to communicate and interact well with hospice team members
- Comfortable with computers and electronic medical records
Qualifications
- Graduate of an accredited school of vocational/practical nursing
- At least one year of experience as an LVN/LPN
Nice to Have
- Experience as a Certified Nursing Assistant (CNA)
- Experience as a Certified Home Health Aide (CHHA)
- Experience as a Medical Assistant (MA)
