This role involves providing community support services to adults with serious mental illness, focusing on linking them to resources like housing and food assistance. You will work directly in the community, managing a caseload and requiring daily local travel.
Responsibilities
- Provide community support services to complex clients, including those on care plans, using treatment plan interventions
- Complete timely documentation of services provided, describing need, intervention, and individual response
- Contact individuals and/or referral sources following missed appointments to re-engage and promote recovery efforts
- Support individuals in crisis situations
- Transport clients in a personal vehicle
- Meet with clients face to face in the community and in their homes
- Connect with all clients in person on a regular basis
Requirements
- High School Diploma or GED
- 2-5 years of experience
- Valid CLASS E or CLASS D driver’s license (or obtain within two weeks)
- Own personal reliable vehicle with valid insurance
- Ability to learn new things quickly and use technology
