Serve as a vital link between patients, healthcare services, and community resources to ensure seamless access to care and improve service delivery. This role involves visiting patients in their homes and Assisted Living Facilities to facilitate provider visits, deliver medications, and durable medical equipment.
Responsibilities
- Work closely with care team members to ensure coordinated patient care and compliance with care plans
- Communicate effectively via phone, email, Microsoft Teams, Athena, and in-person
- Facilitate and coordinate visits with physical therapists, occupational therapists, and providers in Assisted Living Facilities and patients' homes
- Deliver medications to patients’ homes, review instructions, and perform medication reconciliation
- Coordinate and facilitate telehealth visits between patients and pharmacists, specialists, therapists, and primary care providers
- Set up training, instruct patients on the use of Remote Patient Monitoring (RPM) equipment, and provide ongoing support and troubleshooting
- Deliver durable medical equipment (DME) to patients in homes and ALFs, ensuring proper assembly and functionality
- Provide training to patients and caregivers on the correct use of DME
- Perform routine venipuncture for laboratory testing and ensure proper specimen handling and labeling
- Collect blood specimens during home or facility visits as needed
Requirements
- Reliable transportation and daily travel required
- Proficient in using technology tools and platforms for remote patient monitoring, telemedicine, and electronic medical records
- Strong organizational and time management skills
- Ability to multitask and prioritize responsibilities effectively
- Ability to read and communicate effectively, with strong written and verbal communication skills
- Basic computer knowledge
- Ability to work independently with minimal supervision and direction
- Good interpersonal and communication skills
- Ability to work flexible hours and travel between offices/facilities
- Licensed driver with insured automobile in good working condition
- Clean driving record and experience operating vehicles in a professional capacity
Nice to Have
- Certified Medical Assistant, Paramedic, or Licensed Nurse (certification/license in good standing)
- Phlebotomy experience or willingness to complete training
- Previous experience in public health or home health
- Experience with DME delivery or logistics operations
