CNA Hospice Aide

About This Role

Provide direct personal care services to hospice patients, ensuring their comfort and well-being while adhering to safety principles and documenting changes in their status. This role involves assisting patients in their homes within the Woburn, Burlington, and Lexington areas.

Responsibilities

• Provide personal care services to patients
• Apply safety principles and proper body mechanics for patient care, including ambulation, transfers, range of motion, and positioning
• Plan and prepare nutritious meals, and assist with feeding as assigned
• Perform homemaking and other environmental services as assigned
• Observe, report, and document any changes in patient's status
• Follow specific written instructions for personal care prepared by a registered nurse according to the plan of care
• Assist patients with prescribed exercises as instructed by a nurse or therapist
• Perform and record simple urine tests for sugar, acetone, and albumin

Requirements

• High school diploma or equivalent
• Current state certification (or registration) as a nurse assistant
• CNA experience within the last two years
• Current CPR certification
• Valid driver's license
• Reliable transportation
• Liability insurance
