About This Role

The Clinical Pharmacist Specialist will provide specialized pharmaceutical care to ambulatory hematology patients within the Department of Pharmacy Hematology/Oncology team at the University of Michigan Rogel Cancer Center, contributing to treatment plans, patient education, and clinical research.

Responsibilities

  • Provide pharmaceutical care to ambulatory patients seen in the hematology clinics as an integral member of the multidisciplinary team
  • Collaborate with providers on crafting and personalization of treatment plans for patients with lymphoma, myeloma, and benign hematology conditions
  • Guide incorporation of new treatments in a standardized evidence-based fashion, including active involvement at tumor board discussions
  • Coordinate care to promote outpatient delivery of treatments for lymphoma, myeloma, and benign hematology
  • Collaborate with inpatient lymphoma, myeloma, benign hematology, and BMT and cellular therapy teams to provide patient centered care
  • Provide independent visits with patients to educate on complex treatment regimens and provide supportive care
  • Practice under a Collaborative Practice Agreement for nausea, vomiting, constipation, and diarrhea and participate in a related patient-reported outcome text messaging program
  • Assure safe and evidence-based, cost-effective use of medications, as well as contribute to formulary review and maintenance
  • Assist in the development of staff educational and training programs within the Pharmacy Department
  • Participate in the didactic and experimental training of pharmacy students and residents as an adjunct faculty member

Requirements

  • Doctor of Pharmacy degree or equivalent
  • Current license to practice pharmacy in the State of Michigan (including a Controlled Substance license), or eligibility to become licensed within 90 days of employment start date
  • Certificate of completion from an oncology residency, fellowship, or equivalent combination of education, training, and experience
  • Experience in clinical pharmacy practice

Qualifications

  • A Doctor of Pharmacy degree or an equivalent combination of education and experience
  • Entry level with experience in clinical pharmacy practice
