The Clinical Pharmacist identifies, prevents, and resolves medication-related problems while providing authoritative information to healthcare professionals, patients, and the public. This role ensures pharmacy services function as an integrated system to provide patient-centered care within Northwestern Medicine, specifically in Cardiology.
Responsibilities
- Interpret medication orders for safety, proper use, and to identify incompatibilities, duplications, allergies, and potential administration errors.
- Monitor medication therapy for significant drug-drug, drug-diet, or drug-laboratory test interactions and intervene when warranted.
- Evaluate and document medication appropriateness for desired therapeutic outcomes and communicate potential problems to prescribers.
- Consult on pharmacokinetics and adverse reactions to healthcare professionals and adjust therapy based on patient condition and lab results.
- Process medication order information into the Electronic Health Record to ensure accuracy of records.
- Maintain a comprehensive pharmacy patient profile including age, weight, allergies, diagnosis, and pharmacokinetic parameters.
- Oversee packaging, labeling, and distribution of medications and monitor scheduled medications on nursing units.
- Compound and dispense pharmaceuticals, including sterile products, chemotherapy, and parenteral nutrition products.
- Verify medication orders before delivery to patient care areas and resolve discrepancies.
- Review laboratory data for appropriate medication use and recommend changes, including microbial culture, sensitivity, and therapeutic drug monitoring.
- Develop and present orientation, education, and training programs for medical, nursing, hospital, and pharmacy staff, as well as students and residents.
- Educate patients and their families about specific medications.
- Respond to emergency situations to assist with medication management.
- Provide accurate and timely medication information to healthcare professionals and patients.
- Maintain the hospital formulary and limit medication usage to formulary items when appropriate.
- Document all clinical activities and interventions, collecting data and maintaining logs as required.
- Communicate professional knowledge effectively in both oral and written forms.
- Participate in quality improvement and medication use review activities.
- Organize, prioritize, and plan workload, working cooperatively with fellow pharmacists.
- Review restocking automated dispensing cabinets (ADCs) and resolve problems.
- Ensure compliance with hazardous waste collection and disposal procedures.
- Maintain proficiency and expertise through continuous professional development.
Requirements
- Active Clinical Pharmacist license in the state of Illinois (IDFPR)
- Bachelor’s Degree in Pharmacy or a Doctor of Pharmacy Degree
Qualifications

Nice to Have
- Training and knowledge of specialty areas (neonatology, critical care, oncology, etc.)
- Completion of a pharmacy residency program or equivalent hospital pharmacy experience
