The Clinical Coordinator RN provides safe, competent, quality patient care based on nursing theory and research for a designated group of patients, utilizing relationship-centered care and the nursing process.
Responsibilities
- Deliver patient care incorporating Relationship Centered Care and nursing process based on theory, research, evidence-based practice, and organizational standards.
- Assess and evaluate patient care needs, applying critical thinking skills in patient care management.
- Integrate relevant assessment and intervention skills in nursing care delivery.
- Collaborate with the interdisciplinary healthcare team in developing, implementing, and evaluating the plan of care.
- Prioritize all aspects of patient care, including teaching, rounding, coaching, planning after hospital care, and delegating.
- Educate patients and caregivers about their plan of care, transition of care, health promotion, and disease prevention.
- Communicate effectively and professionally with patients, family, and the Interdisciplinary Patient Care Team.
- Manage assignments within the Modified Primary Nursing Care Delivery Model and allocate material resources effectively.
- Initiate service recovery for patients and family members with concerns and escalate issues as needed.
- Practice safe and efficient patient-centered care, identifying and escalating barriers and refinement opportunities.
- Perform charge nurse duties as assigned, coordinating resource utilization with management.
- Advance clinical competence in nursing practice and serve as a professional role model and preceptor.
Requirements
- Graduation from an accredited Nursing program
- Relevant clinical competence in assigned nursing practice area
- Licensed as a registered nurse with current registration in New York State
- Basic Life Saver (BLS) certification from AHA
Qualifications
- Bachelor’s Degree in Nursing (BSN) or Associate’s Degree in Nursing (ADN) RN with matriculation in a BSN Program with a graduation date within the year.
- Relevant clinical competence in area of nursing practice assigned; new graduates must possess current knowledge of the nursing process and its application
Nice to Have
- Associate’s Degree in Nursing (ADN) RN matriculated in a BSN Program with a graduation date within the year
- NRP certification for Labor and Delivery and NICU departments
- ACLS certification for specified critical care and oncology departments
- PALS certification for ED, PACU, IR, Pediatric ICUs, and Pediatric Oncology APNs
