The Pharmacy Technician assists in performing paraprofessional tasks to ensure high-quality pharmacy services, specifically focusing on medication reconciliation.
Responsibilities
- Review and update patients' medication records to ensure accuracy
- Collect and organize patients' medication histories
- Identify discrepancies between medication orders and actual usage
- Consult with healthcare professionals to clarify discrepancies and make necessary changes
- Explain medication instructions and changes to patients or caregivers
- Assist in the transition of care by ensuring accurate medication lists during admission and transfer
- Support the pharmacist to ensure prescriptions are filled correctly
- Document interventions and outcomes of the medication reconciliation process
- Maintain patient confidentiality in accordance with healthcare regulations
Requirements
- High School Diploma or Equivalent
- Registered technician with the Texas State Board of Pharmacy or Pharmacy student current Pharmacy Intern License
- Ability to maintain confidentiality and patient privacy standards
Qualifications
- High School Diploma or Equivalent
- 1 year of Pharmacy experience preferred; Hospital Pharmacy experience preferred
Nice to Have
- 1 year of Pharmacy experience
- Hospital Pharmacy experience
- Computer skills
