About This Role

Assist professional nursing staff by performing patient care activities for veterans in a long-term care setting, ensuring high-quality service and contributing to continuous improvement initiatives.

Responsibilities

  • Assist Veterans/Members with transfers, ambulation, eating, bathing, dressing, toileting, personal hygiene and other Activities of Daily Living (ADLs)
  • Follow instructions to use simple procedures and devices to strengthen Veterans'/Members' limbs and maintain flexibility
  • Assist nursing staff with simple medical procedures such as suppositories, enemas, treatments, post-surgical care and post-mortem care
  • Monitor physical and mental status of Veterans/Members by utilizing appropriate medical equipment and techniques to take vital signs; obtain urine, sputum or stool samples; and track input/output
  • Perform medical record and worksheet documentation on all assigned Veterans/Members, keeping records and preparing simple reports
  • Assist Veterans/Members with dining, including transporting to the dining room, informing about menu choices, communicating with dining staff, and serving meals
  • Provide resident care and comfort by responding to patient requests (verbal or call light) for assistance timely in accordance with the Resident Bill of Rights
  • Maintain cleanliness of areas by changing bedding, linens and other items
  • Report to and assist licensed nurses in emergency situations rapidly and appropriately following established policies and procedure
  • Perform other related duties such as transporting Veterans/Members to appointments outside the facility and clerical duties (e.g., answering phones, filing, etc.) as needed
  • Actively engage in AMS processes such as Huddle Boards, Problem Solving, Job Breakdown Sheets, Success Tracking, and Standardized Work
  • Participate fully in the QAPI process, Quality Assurance and Performance Improvement program mandated by CMS which incorporates AMS

Requirements

  • Current or temporary certification to practice as a Certified Nurse Assistant in the State of Arizona
  • Current Negative TB skin test (Mantoux skin test) within the last twelve months or a written statement from a physician, physician’s assistant or a registered nurse practitioner indicating freedom from Tuberculosis, if in the past has had a positive skin test or Tuberculosis
  • Ability to obtain and retain a fingerprint clearance card issued by the Arizona Department of Public Safety
  • Current CPR card that was obtained by physical assessment

Qualifications

  • One or more years of skilled nursing facility or long-term care experience preferred
