The Care Team Partner (CTP) supports registered nurses by documenting patient information in electronic or written charts and performing patient care duties. This role ensures high-quality health care by assisting with vital signs, daily living activities, and maintaining quality and safety measures.
Responsibilities
- Complete documentation on the RN's behalf, including patient history, assessments, reevaluations, CTP admission and discharge notes, procedures, and medical decision-making verbalized with the RN
- Assist in the delivery of patient care by performing patient care tasks under the direction of a RN appropriate to age, mobility, acuity, and culture
- Respond to patient call bells, obtain and record vital signs, assist with activities of daily living (ADLs) such as bathing, feeding, repositioning, intake, output, indwelling urinary catheter care, and blood glucometer checks
- Place patients on telemetry and transport patients between units or accompany the RN to transport patients to specialty or critical units
- Follow Centers for Disease Control and Prevention (CDC) isolation guidelines, demonstrate appropriate use of Personal Protective Equipment (PPE), perform handwashing according to policy, and enforce safety regulations
- Exhibit a desire to learn and upgrade skills and perform other duties as assigned or directed to ensure smooth operation of the department or unit
Requirements
- High School/GED
- BLS Basic Life Support – American Heart Association
Qualifications
- High School/GED (required), 1 year of undergraduate coursework in pre-nursing or other pre-health (preferred)
- 2 years of comparable experience as Patient Care Technician, State Tested Nursing Assistant (STNA), or medical scribe (preferred)
Nice to Have
- 1 year of undergraduate coursework in pre-nursing or other pre-health area of study
- 2 years of comparable experience as Patient Care Technician, State Tested Nursing Assistants (STNA), or medical scribe
- State Tested Nursing Assistant Licensure
- Certified Medical Scribe
