The Care Manager assesses the health, functional, and cognitive status of clients, primarily older adults and people with disabilities, to determine their eligibility for and need for home care services. This role involves completing assessments, educating clients on resources, and working with their support system to secure necessary services.
Responsibilities
- Complete all necessary annual assessments and associated paperwork to continue client services
- Educate clients regarding available resources and program services
- Work with the client and/or their support system to complete required applications for supportive services and programs
- Monitor clients and/or their support system to evaluate the effectiveness of the plan of care, adjusting as needed
- Complete all required documentation on time under Agency and State contract guidelines
- Prepare any required or requested reports
- Meet critical deadlines established by the funding source's internal procedures
Requirements
- Excellent interpersonal and organizational skills
- Experience with community resources
- Experience with documentation requirements for state-funded programs
Qualifications
- Bachelor's (B.S.) degree in health, social work, gerontology, nursing, or related field
- Minimum of one year of experience working with older adults or those with disabilities
