Care Manager

About This Role

This role involves managing complex care for 60-70 youth in DCFS care in Illinois, focusing on mental and behavioral health needs. You will develop and coordinate personalized care plans, ensuring access to necessary services and providing education to members and their families.

Responsibilities

• Manage 60–70 youth in DCFS care
• Complete required assessments for behavioral and physical health needs
• Create individual care plans with the Interdisciplinary Care Team (ICT)
• Provide ongoing care coordination and follow-up
• Use clinical expertise to maximize health plan benefits and support long-term stability and wellness
• Evaluate member needs via phone or in-home visits and facilitate care plans
• Develop ongoing care plans for members with high acuity, identifying providers and community resources
• Coordinate between members, family/caregivers, community resources, and the care provider team to ensure accessible services
• Monitor care plans/service plans and member status for changes and provide recommendations
• Collect, document, and maintain member information and care management activities for compliance
• Provide education to members and their families on procedures, treatment options, and healthcare benefits

Requirements

• DRS Waiver experience
• Master's degree in Behavioral Health or Social Work or a Degree from an Accredited School of Nursing
• 2 – 4 years of related experience
• Independently licensed behavioral health clinician (e.g. LCSW, LCPC, PsyD) in Illinois or Bachelor’s Degree and IL RN licensure for Illinois Youth Care plan only
• Must reside in IL

• 2-4 years of related experience
