About This Role

The Advanced Practice Professional (APP) will provide high-quality patient care in the Outpatient Nephrology Clinic under the supervision of medical director and attending nephrologists. This role involves performing detailed patient histories, physical examinations, and coordinating care plans with an interprofessional team. The APP will also monitor lab results, provide patient education, and participate in quality improvement efforts.

Responsibilities

  • Work under the supervision of the medical director and attending nephrologists to provide patient care in the outpatient nephrology clinic.
  • Perform and document a detailed patient history and focused physical examinations.
  • Work collaboratively with interprofessional teams to coordinate Plans of Care.
  • Order appropriate tests & procedures, and/or making appropriate referrals.
  • Coordinate care with other medical providers both inside and outside of Michigan Medicine.
  • Monitor labs and assist in the development of patient-specific, facility-based disease management protocols.
  • Advise patients of lab results and provide patient education.
  • Participate in program QA/CQI efforts.

Requirements

  • 2 years of experience working in Nephrology in either an inpatient or outpatient setting as an APP.
  • Experience working with patients who have Chronic Kidney Disease.
  • Current licensure as a Registered Nurse in the State of Michigan (NP) or Physician Assistant in the State of Michigan (PA).
  • Current specialty certification as a Nurse Practitioner (NP) or Certification by the National Commission on Certification for Physician Assistants (NCCPA) for PAs.
  • Current DEA registration.
  • Current AHA Approved BLS certification.

Qualifications

  • Graduate of an accredited nurse practitioner program OR Satisfactory completion of a Physician Assistant program accredited by Accreditation Review Commission on Education for the Physician Assistant (ARC-PA).
  • 2+ years of experience working in Nephrology in either an inpatient or outpatient setting as an APP (Nurse Practitioner or Physician Assistant).

  • Demonstrated ability to prioritize assignments and manage a busy work environment.
  • Ability to function independently and as a team member.
  • Demonstrated communication and customer service excellence with patients, family, staff and physicians.
  • High level of critical thinking and medical knowledge by using current medical information and scientific evidence for patient care.
  • Demonstrated ability to collaborate clinical care ideas across multiple patient care services and to communicate assertively as a patient advocate.
  • Demonstrated ability to carry out professional responsibilities, adherence to ethical principles and sensitivity to diversity.
  • Experience as participant in continuous quality improvement.
  • Computer skills including but not limited to electronic medical record documentation and electronic order entry.
  • EPIC/MiChart experience.
