Join Agape Care Group as a Hospice Liaison, coordinating patient referrals and building relationships with local hospitals to secure new hospice business. You will also educate the community and medical professionals on hospice services, ensuring continuity of care and proper documentation for patients and their care teams.
Responsibilities
- Coordinate patient referrals and generate new hospice business in existing and new accounts
- Identify new markets and maintain existing service
- Serve as a liaison between the agency, hospitals, medical community, and other referral sources
- Educate the community and medical profession/referral sources regarding hospice services
- Interact daily with patients, medical professionals, and other referral sources to assure continuity of care
- Coordinate appropriate communication and documentation for patient care
Requirements
- Associate Degree or equivalent work experience in health-related field
- 2 years of sales experience in a clinical care setting
- Reliable transportation
- Ability to sit, stand, bend, move intermittently and lift at least 25lbs
Qualifications
- Minimum of an Associate Degree in a field of study or proven work experience in a health related field
- 2 years of sales experience in a clinical care setting, hospice preferred
