Treatment Foster Care Coordinator

About This Role

This role is a community-based position that coordinates and monitors clinical services for youth and families, working closely with various stakeholders to provide advocacy and support for successful community living.

Responsibilities

• Provide appropriate individual, group, and family mental health services, and other evidence-based clinical services.
• Travel daily to provide community-based services to youth and families.
• Coordinate and monitor clinical services for youth and family, including mental health, special education, medication management, and psychological services.
• Regularly review and modify youth-based plans.
• Establish and maintain strong relationship with foster parents and families of origin.
• Assist in family reunification and stabilization efforts.
• Provide safety assessment, crisis response, and behavioral stabilization services.

Requirements

• A Bachelor’s Degree in Social Work from an accredited institution
• Valid driver’s license
• Reliable personal transportation
• Good driving record
• Minimum automobile insurance coverage of $100,000/$300,000 bodily injury liability

Qualifications

• A Bachelor’s Degree in Social Work from an accredited institution.
• 2 years of work experience working in direct service with youth and families preferred.

Nice to Have

• LSW license-eligible
• 2 years of work experience working in direct service with youth and families
