This role is a community-based position that coordinates and monitors clinical services for youth and families, working closely with various stakeholders to provide advocacy and support for successful community living.
Responsibilities
- Provide appropriate individual, group, and family mental health services, and other evidence-based clinical services.
- Travel daily to provide community-based services to youth and families.
- Coordinate and monitor clinical services for youth and family, including mental health, special education, medication management, and psychological services.
- Regularly review and modify youth-based plans.
- Establish and maintain strong relationship with foster parents and families of origin.
- Assist in family reunification and stabilization efforts.
- Provide safety assessment, crisis response, and behavioral stabilization services.
Requirements
- A Bachelor’s Degree in Social Work from an accredited institution
- Valid driver’s license
- Reliable personal transportation
- Good driving record
- Minimum automobile insurance coverage of $100,000/$300,000 bodily injury liability
Qualifications
- A Bachelor’s Degree in Social Work from an accredited institution.
- 2 years of work experience working in direct service with youth and families preferred.
Nice to Have
- LSW license-eligible
- 2 years of work experience working in direct service with youth and families
