About This Role

This role involves providing direct support, skill development, and community resource linkages to residents with psychiatric, substance use disability, and/or legal system involvement. The primary goal is to enable residents to live independently and achieve their full potential.

Responsibilities

  • Provide direct support, skill development, and community resource linkages to residents with psychiatric, substance use disability, and/or legal system involvement
  • Enable residents to live independently and work toward realizing their full potential
  • Work mainly in the field (70-80%) meeting with residents on their service plans and assessing their needs
  • Complete progress notes conjointly at the time of service, complying with regulatory standards
  • Support residents with the 8 dimensions of wellness, including housing, emotional health, physical health, social, employment, and educational goals
  • Participate in the development and delivery of service plans
  • Advocate for and develop community-based support
  • Participate in rotating weekends on call by phone

Requirements

  • Bachelor’s degree in a mental health related discipline or High School Diploma or GED plus 3+ years of related work or life experience
  • Ability to write clear, concise, and accurate billable progress notes
  • Valid NJ Driver’s License with acceptable driving record
  • Excellent communication, assessment, and people skills
  • Prominent level of patience and emotional resilience
  • Ability to remain calm under pressure when dealing with challenging situations
  • Ability to carry out domestic duties
  • Ability to work with individuals with mental illness in a caring and professional manner
  • Proficient in MS Office products as well as general computer literacy

Qualifications

  • Bachelor’s degree in a mental health related discipline or High School Diploma or GED plus 3+ years of related work or life experience
  • Entry-level position; 3+ years related experience required if no Bachelor's degree
