About This Role

Supervise and lead designated Case Management teams across multiple sites, providing guidance, coaching, and performance assessment to ensure exceptional, patient-centered care. Coordinate quality initiatives and process improvements within the designated areas.

Responsibilities

  • Provide day-to-day oversight of designated teams across multiple sites
  • Provide leadership, guidance, counseling, and continuing education to staff
  • Select, develop, coach, mentor, and assess performance of staff
  • Assist with coordination of schedules to ensure adequate staffing levels
  • Collaborate with and support area leadership in problem-solving and utilization of resources
  • Coordinate quality initiatives and process improvements for designated areas
  • Track and analyze performance metrics and reports

Requirements

  • Associate of Nursing, Bachelor's of Nursing, or Master's of Social Work
  • 3-5 years of experience depending on education level (5 years for ASN, 3 years for BSN/MSW)
  • Must hold a license in the state of practice as an RN or Licensed Clinical Social Worker (LCSW)

Qualifications

  • Associate of Nursing, Bachelor's of Nursing, or Master's of Social Work
  • 5 years' experience (ASN) or 3 years' experience (BSN/MSW)
