Supervise and lead designated Case Management teams across multiple sites, providing guidance, coaching, and performance assessment to ensure exceptional, patient-centered care. Coordinate quality initiatives and process improvements within the designated areas.
Responsibilities
- Provide day-to-day oversight of designated teams across multiple sites
- Provide leadership, guidance, counseling, and continuing education to staff
- Select, develop, coach, mentor, and assess performance of staff
- Assist with coordination of schedules to ensure adequate staffing levels
- Collaborate with and support area leadership in problem-solving and utilization of resources
- Coordinate quality initiatives and process improvements for designated areas
- Track and analyze performance metrics and reports
Requirements
- Associate of Nursing, Bachelor's of Nursing, or Master's of Social Work
- 3-5 years of experience depending on education level (5 years for ASN, 3 years for BSN/MSW)
- Must hold a license in the state of practice as an RN or Licensed Clinical Social Worker (LCSW)
Qualifications
- Associate of Nursing, Bachelor's of Nursing, or Master's of Social Work
- 5 years' experience (ASN) or 3 years' experience (BSN/MSW)
