Responsibilities
- Conduct Start of Care (SOC) visits
- Complete OASIS documentation accurately and timely
- Perform skilled nursing assessments
- Educate patients and caregivers
- Collaborate with interdisciplinary teams
- Ensure compliance with care plans and regulations
Requirements
- Active Registered Nurse (RN) license in the state of Michigan
- Minimum of one year of nursing experience, preferably in home health care or acute care setting
- Strong assessment and documentation skills
- Reliable transportation
- Positive energy, compassion, and professionalism
- Excellent communication and organization
Qualifications
