Admissions Hospice Registered Nurse

About This Role

Provide compassionate one-on-one hospice care to patients in their homes, supporting them and their families with dignity during their final days. This role emphasizes presence and honoring each moment, ensuring gentle and personal care.

Responsibilities

• Provide one-on-one care in the comfort of home
• Support patients and families with dignity and heart
• Provide emotional support to help patients live their final days with dignity and comfort
• Conduct appropriate supervisory visits for clinical observation of assigned team members
• Participate in and provide feedback for interdisciplinary group meetings to develop, review, and revise patient/family plans of care
• Conduct initial and on-going assessment of patient's physical, functional, psychosocial and environmental needs
• Implement the individualized plan of care and make revisions as needed
• Consult with and educate the patient/family on disease process, self-care techniques, end-of-life care, and ethical concerns
• Supervise paraprofessionals providing services to patients
• Prepare clinical and progress notes and participate in IDG conferences
• Participate in Hospice performance improvement program
• Maintain high level of customer satisfaction with clients, families, and caregivers
• Communicate information using current process and technology within the organization

Requirements

• Current Michigan License in good standing
• Minimum 1 year hospice experience
• Empathetic approach to patients and caregivers
• Ability to adapt to patient's status and needs on a full time on call basis

Qualifications

• Minimum 1 year hospice experience
