Hospice Chaplain - Pastoral/Spiritual Counselor

About This Role

Provide spiritual and pastoral care to hospice patients, families, and caregivers, developing individualized care plans and collaborating with clergy. Participate in home visits, team meetings, and continuous improvement initiatives for Hospice Services.

Responsibilities

• Conduct spiritual/pastoral assessment of patient's, family and caregiver spiritual needs
• Develop individualized spiritual plans of care, collaborating with local clergy when desired by patient
• Provide spiritual support as needed and defined by patient/family
• Ensure compliance with AdvisaCare Hospice Policies and Procedures and Medicare regulations
• Prepare clinical and progress notes
• Participate in Interdisciplinary Group (IDG) conferences
• Be available to staff for bereavement support
• Participate in the Hospice performance improvement program
• Maintain high level of customer satisfaction with clients, families, and caregivers
• Consistently and accurately update Electronic Medical Records

Requirements

• Graduate of accredited seminary or school of theology or appropriate certification of pastoral ministry
• Demonstrated experience in working with patients and families who have dealt with loss of a loved one
• Empathetic approach to patients, caregivers and team members
• Ability to adapt to patient's status and needs
• Exhibits critical thinking and applies them for continuous improvement of Hospice Services

Qualifications

• Graduate of accredited seminary or school of theology or appropriate certification of pastoral ministry
