Home Health and Hospice Registered Nurse

About This Role

Provide compassionate one-on-one home health and hospice care to patients. Support patients and families with dignity, focusing on comfort and meaningful moments in their final days.

Responsibilities

• Provide one-on-one care in the comfort of patients' homes
• Support patients and families with dignity and compassion
• Provide emotional support to help patients live their final days with dignity and comfort
• Conduct appropriate supervisory visits for clinical observation of team members
• Participate and provide feedback in interdisciplinary group meetings
• Assist in the development, review, and revision of patient/family plans of care

Requirements

• Current Michigan Registered Nurse (RN)
• Updated Tuberculin (TB) test
• Current CPR certification
• Clear Criminal History/Drug Screen
• Compassionate and caring demeanor, with a professional presentation
• Ability to travel within service area of AdvisaCare (approximately a 30 mile radius from your home)
• Critical thinking, task competence and the ability to think on your feet

Qualifications

• Minimum one (1) year of Home Health or Hospice nursing experience preferred, but not required.
