Provide compassionate one-on-one home health and hospice care to patients. Support patients and families with dignity, focusing on comfort and meaningful moments in their final days.
Responsibilities
- Provide one-on-one care in the comfort of patients' homes
- Support patients and families with dignity and compassion
- Provide emotional support to help patients live their final days with dignity and comfort
- Conduct appropriate supervisory visits for clinical observation of team members
- Participate and provide feedback in interdisciplinary group meetings
- Assist in the development, review, and revision of patient/family plans of care
Requirements
- Current Michigan Registered Nurse (RN)
- Updated Tuberculin (TB) test
- Current CPR certification
- Clear Criminal History/Drug Screen
- Compassionate and caring demeanor, with a professional presentation
- Ability to travel within service area of AdvisaCare (approximately a 30 mile radius from your home)
- Critical thinking, task competence and the ability to think on your feet
Qualifications
- Minimum one (1) year of Home Health or Hospice nursing experience preferred, but not required.
