This role provides social work services to participants in a Program of All-Inclusive Care for the Elderly (PACE) program, focusing on direct service activities like counseling and assisting with social issues. The Social Worker will empower elders in the community to maintain active, independent lives.
Responsibilities
- Provide social work services to participants in the PACE program
- Engage in direct service activities within the Center
- Conduct one-on-one and group counseling
- Interact with other interdisciplinary team members
- Provide ongoing assistance to participants to address social issues
Requirements
- Master's Degree in Social Work
- Washington Clinical Independent Social Worker Associate or Independent License (or Temporary)
- 1 year of experience with geriatric population
- 2 years of social work experience with geriatric age groups from diverse ethnic cultures
Qualifications
- Master's Degree - Social Work
- 1 year of work experience with geriatric population, 2 years of social work with geriatric age groups from diverse ethnic cultures.
