About This Role

The Community Care RN provides compassionate, independent nursing care to Providence patients within the PACE program, focusing on promoting optimal health and independence for older adults living in the community.

Responsibilities

  • Provide and supervise professional, independent nursing care to Providence patients
  • Apply knowledge of assessing, planning, ordering, giving, delegating, teaching, and supervising professional nursing care
  • Deliver care consistent with the Oregon Nurse Practice Act
  • Promote patients' optimal health and independence
  • Empower elders in the community to live active, independent lives

Requirements

  • Oregon Registered Nurse License
  • National Provider BLS - American Heart Association (within 30 days of hire)

Nice to Have

  • Bachelor's Degree in Nursing
  • 1 year Home health RN experience
