Social Worker MSW

About This Role

Implement and maintain social work services standards for Home Health and Hospice patients. Assess psychosocial status, provide psychosocial interventions, and connect clients with community resources.

Responsibilities

• Assess the psychosocial status of clients related to their illness and environment and communicate findings to the registered nurse
• Carry out social evaluations and plan interventions based on evaluation findings
• Maintain clinical records on all clients referred to social work
• Provide information and referral services to clients and families/caregivers regarding environmental needs
• Serve as liaison between clients or families/caregivers and community agencies
• Maintain collaborative relationships with organization personnel to support client care
• Participate in the development of the total plan of care and case conferences as required
• Assist medical team in understanding significant social and emotional factors related to health problems
• Participate in discharge planning
• Coordinate new volunteer training, continued education, and annual evaluations
• Assign volunteers to each hospice family and coordinate visits
• Coordinate the bereavement program
• Be the contact person with the foundation department at the local hospital in conjunction with the agency director
• Be responsible for event coordination, planning and execution

Requirements

• Bachelor's degree in Social Work
• Licensed as a Social Worker in Minnesota and North Dakota
• One year experience in healthcare
• Excellent verbal, written communication skills
• Organizational skills
• Basic computer skills
