Social Worker - Hospice

About This Role

This Social Worker role for Hospice in Grand Island, NE, is responsible for providing direct social services to Seniors/clients and their families, addressing emotional and social needs, and ensuring compliance with State and Federal regulations.

Responsibilities

• Implement social service programs to meet medically related emotional and social needs of Seniors/clients and their families
• Assist Seniors/clients with problem solving, crisis counseling, bereavement counseling, information resources, and referrals
• Participate in discharge planning, complete Senior/client assessments, and develop/implement care plans
• Develop working relationships with Seniors/clients, families, and staff to understand social and emotional needs
• Serve as a liaison and advocate for Seniors/clients and their families
• Provide information regarding financial concerns, including Medicare, Medicare Part D, Medicaid, and other assistance programs
• Complete assessments and documentation as required by state/federal regulatory requirements and department policy

Requirements

• Bachelor’s degree from an accredited school of social work
• One year of clinical experience with terminally ill or medically ill clients
• Certified by the State of Nebraska as a bachelor’s or master’s level social worker
• Valid Nebraska Driver’s license and good driving record
• Auto insurance at or above Nebraska statutory requirements
• Ability to speak, read, write, and understand the English language

Qualifications

• Bachelor’s degree from an accredited school of social work, required (casework to be supervised by an MSW). Graduate Masters from an accredited school of social work, preferred.
• One year of clinical experience appropriate to the counseling and casework needs of the terminally ill and/or medically ill required.
