RN - Registered Nurse - Hospital Case Manager

About This Role

This RN Hospital Case Manager position ensures patients receive appropriate care, optimizes health outcomes, and manages utilization through evidence-based practices at Geisinger Medical Center.

Responsibilities

• Integrate evidence-based clinical guidelines and protocols into treatment plans
• Develop systems of care to monitor progress and enable early intervention in acute cases
• Assist in the design, implementation, and evaluation of advanced patient-centered care models
• Assess healthcare, educational, and psychosocial needs of patients
• Design individualized plans of care and foster a team approach with patients, families, and healthcare providers
• Continuously evaluate laboratory results, diagnostic tests, and utilization patterns
• Apply benefits and utilization management, serving as a resource to the patient and healthcare team
• Maintain required documentation for all case management activities
• Collect and utilize data to adjust treatment plans as indicated
• Assess, plan, implement, coordinate, monitor, and evaluate all options and services to optimize patient health status

Requirements

• Minimum of 2 years of RN experience
• Valid Pennsylvania Registered Nurse license

Qualifications

• Graduate from Specialty Training Program-Nursing (Required), Bachelor's Degree-Nursing (Preferred)
• Minimum of 2 years of nursing experience; prior RN experience in a hospital setting preferred.
