RN Navigator - Maternal Fetal Center

About This Role

This RN Navigator role is for a specialty nurse who will function as an expert clinician, educator, consultant, and care coordinator within the Maternal Fetal Center, collaborating with the healthcare team to improve patient access and outcomes.

Responsibilities

• Function as an expert clinician, educator, consultant and coordinator of care
• Coordinate, monitor and evaluate clinical care and services for patients and families
• Provide clinical support to improve access to care, patient care, and outcomes
• Serve as a resource and primary contact to assist patients and families in navigating healthcare and treatment
• Assist patients, families, staff, and physicians in finding solutions for complex patient care issues
• Coordinate activities across the continuum of care, interdepartmentally, and with multiple healthcare facilities and subspecialty providers

Requirements

• 2 year College Degree in Nursing
• RN - Registered Nurse License - CA-BRN

Qualifications

• 2 year College Degree in Nursing (required) or 4 year College Degree in Nursing (preferred)
• Minimum two (2) years experience in clinical nursing specialty (preferred)

Nice to Have

• 4 year College Degree in Nursing
• Minimum two (2) years Experience in clinical nursing specialty
• Extensive advanced knowledge of nursing principles, clinical practice, and healthcare systems
