RN Charge Nurse

About This Role

The RN Charge Nurse provides one-on-one home visits to patients, conducting initial and ongoing assessments, administering medication, and educating patients and families on proper home health care strategies. This role requires excellent communication and a compassionate demeanor to ensure high-quality healthcare and customer service.

Responsibilities

• Visit patients in their homes for one-on-one visits
• Complete initial and ongoing assessments to identify physical, psychosocial, and environmental needs
• Assess patients' conditions, chart observations, and perform evaluation tasks including vital signs and medication review
• Administer medication as prescribed by the patient's physician
• Educate patients and their families on proper home health care strategies and procedures
• Provide effective communication to patients/clients, their family members, team members, and other health care professionals

Requirements

• Current state license as an RN
• At least one year experience in skilled/long term care environment
• Proof of current CPR certification
• Reliable transportation
• Ability to work a flexible schedule and travel locally

Qualifications

• Diploma, Associate, or Bachelor’s in Nursing
• At least one year experience in skilled/long term care environment
