RN Case Manager

About This Role

The RN Case Manager will provide comprehensive home healthcare services, including assessment, planning, and evaluation of patient care, ensuring coordinated services with physicians and families, and supervising home health aides to deliver care in the patient's home.

Responsibilities

• Assess physical, emotional, socio-economic, and environmental needs of patients and their families
• Provide physician-ordered services as outlined in the plan of care
• Communicate with all physicians and healthcare practitioners involved in the plan of care
• Develop, implement, and evaluate the plan of care in partnership with the patient, representative, caregiver, and physician(s)
• Coordinate services with the patient, family, and others involved in the care plan
• Provide patient, caregiver, and family counseling and education
• Regularly evaluate patient progress and take prompt action for changes in condition
• Participate in agency quality assurance, performance improvement, and in-service training programs
• Identify patient and family needs for other home health services and make appropriate referrals
• Supervise services rendered by homemaker/home health aides according to State and Federal guidelines and Joint Commission standards
• Prepare clinical notes and attend in-service training to fulfill compliance responsibilities

Requirements

• Current RN License in Connecticut
• BLS certification
• Minimum one year experience as an RN
• Strong research skills
• Clear and concise interpersonal communication skills
• Knowledge of systems integration

Qualifications

• RN diploma or Associates Degree
• Minimum one year of experience as an RN, or over 3 years job related experience with an RN diploma in lieu of an Associate's Degree

Nice to Have

• Experience in a qualified home health agency
